甲方僱用乙方於中華民國境內擔任 工作而簽訂本合約。雙方約定事項有關條件詳列於下:
Whereas, the Employer is desirous of employing the Employee to work as a for the Employer in Taiwan, R.O.C.; Now therefore the two parties mutually agree to enter into this Employment Contract as follows.
第一條 :契約期間
Article 1:Term of Contract
自乙方抵達中華民國台灣地區報到之日起年月日(可展延),並以最初之四十日為試用期間。期滿甲方徵求乙方同意後,並依中華民國政府法令規定,得重新商定勞動契約延期一年者外,期滿勞僱關係自動終止。
This Contract shall be valid for a period ofyear(s)month(s)day(s) (Renewable), commencing from the date when Employee arrives in Taiwan and reports to duty, and the first forty (40) days therefrom shall be the probation period. Unless otherwise agreed by Employee and approved in accordance with the Laws and regulations of the R.O.C. to have the Employment Contract renewed for a further period of one (1) year, this contract shall be automatically terminated upon expiration hereof.
第二條 : 工作義務
Article 2:Obligations
乙方接受甲方監督指揮,擔任甲方所指定之工作,並應保持良好態度,妥善維護本人及同事之安全。
Under the supervision and direction of Employer, Employee shall take a good attitude to do the job as designated by Employer and properly keep himself/herself and other associates safe to maintain good manners.
第三條:工作時間
Article 3: Work Time
每日正常工作時間八小時,每週工作六日。如有超工作或晝夜輪班需要時,乙方應配合甲方要求依中華民國勞動基準法令辦理。
Employee shall work eight (8) hours per day, six (6) days per week normally. In the event that over-time working or day/night shift is required, Employer should comply with. This over-time working or day/night shift should be subject to the regulation of the labor Laws of R.O.C.
第四條:工作報酬
Article 4: Remuneration
4.1 工資:月支工資為新台幣元,4.2 每月定期發給並依中華民國有關法令規定由甲方代表為扣繳工資所得稅及勞健保費用自繳部分。
Salary:Monthly salary is NT$ , payable on a fixed date each month, from which Employer may withhold the income tax and deduct the portion of health labor insurance premium to be borne by Employee in accordance with the Laws and Regulations of the R.O.C.
4.2超時工作報酬計算方式,依中華民國勞動基準法。
Overtime payment shall be computed in accordance with the Labor Standards Law of R.O.C.
4.3 甲方提供乙方膳宿,但每月自薪資中扣新台幣 元膳宿費。
Employer provide the employee food and accommodation and deduct NTD from salary per month.
4.4 乙方自付前往中華民國及服務期滿後返國之機票費用。
Employee shall pay with economy class air ticket from Indonesia to R.O.C. and after Employee has completed Employment with Employee back to Indonesia.

第五條:休假及病假
Article 5: Leave or sick Leave
5.1 乙方於服務每滿一年由甲方給予特別休假七日,或折發工資。
In case Employee has worked for a full year, employer shall grant Employee a special vacation for seven (7) days which may be converted into cash.
5.2 乙方因普通傷病需請假者依中華民國勞動基準法之勞工請假規則規定辦理。
Employee shall have sick leave in case of sickness or injury in accordance with the Regulation of the Labor Standards Law of the R.O.C.
5.3 乙方每七日中享有一天休假,每年並享有11項之國定假日如下:
Employee shall have one day off in every seven days, and have following 11 national holidays every year:

第六條:勞工保險
Article 6: Labor Insurance
於聘僱期間內,甲方應為乙方辦理加入勞工及健康保險,保險費之負擔則依中華民國勞動基準法及相關法令分攤之。
During the period of employment, Employee shall join the labor insurance, for which the payment of insurance premium and the claim of insurance benefits shall be made in accordance with the provisions specified in the Labor Insurance Act of the R.O.C.
第七條:契約之終止及效果
Article 7: Termination and Its Effect
